In July 2008 Mr Chodkiewicz a 53 year old man of Bristol was working on a lift installation. Mr Chodkiewicz was at the bottom of the lift shaft and a co-worker was 18 storeys; up, at the top of the shaft.

A piece of 7lb scaffolding tube with piano wire around it was being raised by the co-worker up the shaft. Whilst raising the scaffolding tube, it got to the twelfth floor when the wire became untied and the scaffolding tube fell onto Mr Chodkiewicz head.

Mr Chodiewicz suffered severe permanent brain damage and will require 24 hour care for the rest of his life.
The Health and safety executive brought prosecutions again two of the companies involved. The case was heard in the Bristol County Court and the Court found that both companies had breached Health and safety and were fined £70,000.00 and £40,000.00.
I feel this accident could of been easily avoided if the task had been risk assessed, and agree with the court this accident shows carelessness on behalf of the two construction firms involved.
