I was saddened to read this morning about 63 year old Philip Pratt. He had been suffering from prostate cancer and was admitted to St Richards Hospital in October 2012 to undergo an operation to his kidneys. Whilst in hospital Mr Pratt suffered a number of falls and fractured his collarbone and hip.

Returning a verdict of accidental death, coroner Elizabeth Bussey-Jones, said: "Although he was a significantly ill man, it is apparent his rapid decline seems to be associated with his falls. Most significant was the last fall which seems to have fractured the neck of femur and the infection of the lungs. The prescription he was on was not sufficient to prevent the onset of pneumonia."

Janie Whittaker, head of nursing at St Richards, apologised to Mrs Blakeborough, and told the inquest nursing staff have adopted procedures to deal with the use of the drug Tramadol.

Whilst the change to procedures at the hospital will be welcomed, it will prove little comfort for Mr Pratt's family.
