I was alarmed to read this week an article titled 'rise in surgery on the wrong body part.'

The article, from the Times newspaper, described how in 2015 there were 135 cases where surgeons operated on the wrong body part. This is in contrast to the figure from four years ago of only 54 cases. The NHS believe that the rise does not mean that more errors are occurring, but rather that trusts are now being more honest when mistakes are made.

Either way it is concerning that mistakes like these are still being made at all. Common excuses seem to be 'distorted anatomy' or 'mix up of consent forms'. Whatever the reason, operating on the wrong body part is a 'never event' and by virtue of its very definition is something that should never happen. It is hoped that trusts will learn from these mistakes and address such errors by providing training or guidance to its staff so that these mistakes do not occur again.
